This increases its capacity as a provider of high-quality medical care, leading to a tertiary care, referral, and teaching hospital.<br \/>\nPhase II development focus areas: Expanding key clinical services including:<br \/>\nA comprehensive cardiology programme<br \/>\nOncology services<br \/>\nNeurosciences<br \/>\nOrthopaedics and trauma<br \/>\nDiagnostic imaging<br \/>\nCritical care<br \/>\nWomen and child health programmes<br \/>\nAccreditations and affiliations:<br \/>\nJCI accredited (achieved a level of quality and patient service equal to the best hospitals in the world)<br \/>\nTeaching site for The Aga Khan University, offering postgraduate medical education programs leading to Master of Medicine in: Family Medicine, Internal Medicine, Surgery, Obstetrics and Gynecology, Paediatrics and Child Health<br \/>\nAccredited internship site<br \/>\nOutreach health facilities: Plans to establish 35 outreach health facilities across Tanzania. Currently, 19 such clinics have been established and linked to the main hospital in Dar es Salaam.<br \/>\nHiring statement: AKHST is seeking enthusiastic, qualified, and experienced personnel as described below.<br \/>\nPosition title: Patient safety officer \u2013 (1 position)<br \/>\nReports to: Ops Manager<br \/>\nPosition summary:<br \/>\nThe Patient Safety Officer is responsible for building and strengthening patient safety and patient-centered culture within AKHST. He\/she coordinates other departments toward process improvements that will support the reduction of clinical errors and other factors that contribute to unintended adverse patient outcomes.<br \/>\nJob Application Services<br \/>\nKey responsibilities:<br \/>\nEnsure that patient safety standards are implemented in patient care areas in accordance with set Joint Commission International (JCI), SafeCare, and hospital standards requirements.<br \/>\nOversee the implementation and compliance with International Patient Safety Goals in patient care areas.<br \/>\nConduct internal clinical audits and participate in external audits.<br \/>\nParticipate in mortality and morbidity reviews, offer guidance in accordance with the institutional policies and set standards.<br \/>\nProvide training, support, and guidance to clinical teams on IPSG standards, institutional policies and procedures to ensure that patient care standards are upheld and implemented.<br \/>\nParticipate in Continuing Medical Education (CME), Continuing Nursing Education (CNE) and medical staff orientation sessions on quality and patient safety standards.<br \/>\nMonthly and quarterly reporting of International Patient Safety Goals (IPSG) compliance and data validation.<br \/>\nWork with the Ops Manager in the development of consistent and organization-wide approaches, policies, programs and action plans from patient safety events\/complaints.<br \/>\nSupport and participate in institutional Joint Commission International Accreditation and other quality tools and standards with appropriate processes and procedures.<br \/>\nPerforms clinical audits to ensure provision of safe patient care as per best practices.<br \/>\nParticipating in and offering guidance in the development, implementation, review of clinical policies and procedures in accordance with evidence-based practice.<br \/>\nMonthly reporting of patient falls and pressure ulcers incidence and facilitating root cause analysis.<br \/>\nSupport operational staff to identify and implement changes to clinical delivery systems to improve patient outcomes.<br \/>\nCoordinates incident review process for patient safety events.<br \/>\nDaily review and monthly logging of all transfer outs\/transfers within the hospital to ensure quality and safety of patient transportation.<br \/>\nConduct monthly ambulance audits.<br \/>\nConduct and coordinate monthly open and\/or closed charts audits and compile data to ensure compliance with clinical documentation standards.<br \/>\nPerform any other duties assigned by the supervisor or his\/her designee.<br \/>\nEducation and experience:<br \/>\nMinimum bachelor\u2019s degree in nursing from a recognized university\/college.<br \/>\nValid registration and practicing license with the Tanzania Nursing and Midwifery Council (TNMC).<br \/>\nRelevant active clinical experience, minimum two (2) years of experience in a hospital environment required.<br \/>\nA background in health management or public health would be advantageous.<br \/>\nEqual employment opportunity (EEO) statement:<br \/>\nAga Khan Health Service, Tanzania (AKHST) is an equal opportunity employer.
