AHF is looking for highly a qualified Linkage and Retention Officers to fill in the above-mentioned positions that have arisen within the organization\u2019s supported facilities. <\/p>\n<p>Position Summary The Linkage and Retention Officer will be assigned to work at an AHF supported heath facility under the guidance and technical supervision of the sister in charge and doctor in charge at the respective facility.<\/p>\n<p>\t\t\t\tDuties and Responsibilities<br \/>\n\t\t\t\tClient Retention &amp; Linkage\u2022\tTrack and follow up clients who miss appointments, default treatment, or are at risk of loss to follow-up.\u2022\tConduct phone calls, SMS reminders, and community\/home visits in line with confidentiality guidelines.\u2022\tMaintain updated client appointment and tracking records.\u2022\tSupport Back to Care clients who have disengaged from care back into services\u2022\tEnsure all linkage and retention operations in different clinics under their jurisdiction are effectively coordinated\u2022\tEnsure the requisite resources are available to facilitate implementation of linkage and retention activities\u2022\tEnsure linkage and retention activities are planned with clear performance targets\u2022\tEnsure that &gt;90% of newly tested positives are linked to care while &gt;90% of all clients both new and old are retained into careAdherence and Psychosocial Support \u2022\tProvide adherence counseling and basic psychosocial support to clients living with HIV.\u2022\tIdentify and address barriers to retention such as stigma, disclosure challenges, transport issues, or side effects.\u2022\tRefer clients to peer support groups, social services, or mental health services where necessary.\u2022\tParticipate in ensuring continued Youth friendly service provision working in partnership with the CATS and all StaffHIV Testing Services\u2022\tFacilitates education on condom use including condom demonstration\u2022\tProvides pre and post HIV confidential counselling and testing, to all patients willing to have the HIV test \u2022\tEnsures that HIV IEC materials are available on testing sitesStigma Reduction and Client Rights\u2022\tPromote client-centered care, confidentiality, and human rights-based approaches.\u2022\tSupport stigma reduction activities within communities and facilities.\u2022\tAdvocate for differentiated service delivery (DSD) models suitable for clients\u2019 needsCommunity and Facility Co-ordination \u2022\tWork closely with health facilities, community health workers, expert clients, and peer educators to support linkage and retention strategies.\u2022\tParticipate in community outreach activities targeting key and priority populations.\u2022\tStrengthen linkages between community-based and facility-based HIV services\u2022\tCo-ordinate linkage and retention in assigned health facilities<\/p>\n<p>Data Management and Reporting \u2022\tAccurately document follow-up outcomes, defaulter tracing, and retention indicators.\u2022\tUpdate registers, electronic databases, and M&amp;E tools as required.\u2022\tPrepare weekly and monthly retention and linkage reports for supervisors.\u2022\tCollect weekly, monthly &amp; quarterly HIV Testing Services and ART Statistics from the respective site, including the composition of the AHF-specific Reports; Enter all weekly, monthly, and quarterly data into the AHF web portal; \u2022\tActively participates in preparing weekly, monthly, and quarterly program data reports and ensures timely submission of requested data to MoHCC and AHF structures, as well as assists in the production of information submitted to the MoHCC DHIS2; \u2022\tSupport data quality improvement initiatives\u2022\tParticipates in routine performance assessment reviews that are conducted quarterly, which focus on performance indicators.Compliance and Quality Assurance \u2022\tAdhere to national HIV treatment guidelines and organizational policies.\u2022\tMaintain confidentiality and ethical standards at all times.\u2022\tParticipate in trainings, review meetings, and program evaluations.<\/p>\n<p>\t\t\t\tQualifications and Experience<br \/>\n\t\t\t\t\u2022\tMust possess a Diploma or its equivalent in either of the following disciplines Psychology, Nursing, Counseling, Social work, administration, public health.\u2022\tThe preferred candidates will be those who also possess a Certificate or its equivalent in either of the following disciplines M&amp;E, Statistics, Data Analysis or Information Technology.\u2022\tHave rapid HIV Testing certificate \u2022\tA minimum of 2 years\u2019 experience working with clients in public health institutions, community services or medical setting. \u2022\tPrevious exposure and experience working with healthcare delivery systems such as eHR, ePMS, ePOC, DHIS 2 is a significant advantage; Flexible, proactive, and able to organize work independently as well as work in a team. \u2022\tShould have strong interpersonal and communication skills both verbal and written.\u2022\tAbility to function under dynamic circumstances and proficiency in computer skills e.g. MS Word, Excel, Outlook &amp;
