Job Description

Job Summary
We are seeking a compassionate, proactive, and community-focused Community Health Worker to join our healthcare team.
The successful candidate will play a key role in promoting health education, disease prevention, and community wellness by providing basic health services, conducting outreach activities, and connecting individuals with appropriate healthcare resources.
The ideal candidate should possess excellent communication skills, a passion for community service, and a commitment to improving health outcomes.
Key Responsibilities
Conduct community outreach programs to promote health awareness and preventive healthcare practices.
Educate individuals, families, and community groups on hygiene, nutrition, maternal and child health, immunization, family planning, and healthy lifestyle practices.
Identify community health needs and provide appropriate health education, guidance, and referrals.
Organize and participate in medical outreaches, health campaigns, wellness screenings, and vaccination programs.
Perform basic health assessments, including blood pressure, blood glucose, temperature, weight, and other routine health checks where applicable.
Support clients in accessing healthcare services by coordinating referrals and follow-up care.
Conduct home visits to monitor clients’ health status and reinforce health education where necessary.
Maintain accurate records of patient interactions, outreach activities, referrals, and community health reports.
Collaborate with healthcare professionals and other stakeholders to ensure continuity and quality of care.
Encourage adherence to prescribed treatments and promote positive health behaviors.
Identify and promptly report disease outbreaks, public health concerns, and emergencies to the appropriate authorities.
Participate in health education seminars, training sessions, and continuous professional development activities.
Ensure confidentiality and privacy of patient and community information at all times.
Comply with organizational policies, infection prevention protocols, and health and safety regulations.
Perform other related duties as assigned by management.
Requirements
Minimum of OND, HND, or Bachelor’s Degree in Community Health, Public Health, Nursing, Health Education, Environmental Health, or a related field.
1–3 years of relevant experience in community health, primary healthcare, public health, or wellness programs.
Valid professional certification or license where applicable is an added advantage.
Sound knowledge of health education, disease prevention, and community engagement strategies.
Good understanding of primary healthcare principles and public health practices.
Excellent communication, interpersonal, and counselling skills.
Strong organizational, documentation, and reporting skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Proficiency in the use of Microsoft Office applications is an added advantage.
Ability to communicate effectively in English and local languages is an added advantage.
